Appointments

Appointments are scheduled Monday through Friday between 7:00 a.m. and 5:00 p.m.
by calling (540)941-2500

We recommend that you arrive 20 minutes prior to your scheduled appointment to complete necessary patient information. For your convenience, you can download the forms from our site and have them ready when you arrive. Also remember to write down any specific questions you may have to ensure all of your concerns are addressed.

In order to best serve all patients, we ask that you give at least 24 hours notice if you will be unable to keep a scheduled appointment.

Payment for Services/Insurance

Payment is expected at the time any services are rendered. If you have any questions regarding billing, please call our office at 540-941-2500. We accept cash, checks, VISA, and MasterCard.

It is extremely important that you be familiar with the requirements of your insurance plan prior to any scheduled visits to the office. Remember to bring your insurance card, photo I.D., referral (if necessary) and co-payment if required to your appointment.

Cancellation Policy

Twenty-four hour’s notice is required for cancellation of scheduled appointments. You are subject to a charge of the following fees for appointments that you “no show” or cancel with less than 24 hour’s notice:

• Initial appointments: $50
• Follow-up appointments: $25
• Therapy appointments: $50
• Testing appointment: $50 per hour

You are at risk of losing your privilege to receive care at Comprehensive Health Systems, PLC if you “no show” or cancel for three or more appointments.

Medication Complications

If you are having medication concerns which require a same-day response, we will make an effort to schedule a same-day appointment with Dr. Kane or Mrs. Judge. Appointments with your regular medication provider may not be available. You may be expected to schedule a same day appointment with a different provider in order to address your medication concerns.

Medication Management Appointments:

Attempts will be made to maintain your appointments with your regularly scheduled medication provider. However, due to emergencies, etc., providers may be called away. You may be asked to see another provider for your scheduled appointment.

Professional Charges

Telephone Consultation
You will be charged for return calls that require provider input. The charge will be dependent on the complexity and/or time involved. Typical fees for return calls range from $7.50 to 22.50 or more per call. You may not be called back the same day. If your concerns require a same-day response, you will need to schedule an urgent appointment.

Fees for Letter Requests

• Simple letter - $15
• Letter of one to two paragraphs - $25
• Complex letter - charge according to length/time involved

Fees for Requested Forms
A charge of $15 will be charged per simple form. This includes disability forms, medication preauthorization forms, work forms, school forms, etc. You may be charged more per form, depending on the complexity and time involved. If additional paperwork is necessary after initial submission, additional fees may be required.

Medical Records Fees
Requests for medical records need approval by the HIPPA compliance coordinator, a federal law. Therefore, medical records cannot be completed on a same-day basis.

The following charges apply to medical records requests:
There will be a $15 flat fee per request plus 25 cents per copied one-sided page and 50 cents per page for a copies over 50 pages. Additional charges for postage, fax, etc apply.
